Step 2: Upload the completed forms to this page, email them to us at contact@lakehillsmontessori.com, or bring them to us in person. Please include the name of your child in the file name for the forms you upload.

Step 3: Pay the application fee on this page, or bring us your payment in person.

You must complete and submit all forms and pay the application fee for your application to be complete. Incomplete applications will not be considered.
